Online Application Forms

The deadline for applications was May 30 and the applications process is closed. Teams will be notified of their participation by the end of July. The forms below are being kept to allow for administrative processing of the applications and to allow teams that are selected to update any personal information that may change. Any applications submitted at this time will not receive consideration.

Application Instructions

On-line forms for applying to participate in the SC2003 Education Program:
One person should complete a Team Form for your team's application. Each team must include faculty members. Each member of the team should complete either a
Faculty Member Form - or a -
K-12 Teacher Form as appropriate to their teaching responsibilities.
