There are a limited number of rooms reserved at the government rate so make your reservation as soon as possible. Submit one form per room (you may photocopy this form). All room reservations for SC'96 must be made through the Pittsburgh SC'96 Housing Bureau. Telephone reservations will not be accepted. Once this form is processed, you will receive an acknowledgement from the housing bureau, followed by a confirmation from the hotel.
